Wisdom Tooth / Third Molar
Oral surgeons perform wisdom tooth removal. It is a regular procedure nowadays.
There are four wisdom teeth or third molars – two in the upper jaw and two in the lower jaw. Wisdom teeth usually start erupting after 18 years of age. Since they erupt after the completion of jaw growth, they usually don’t have sufficient place to erupt. So, many times they remain as trouble makers in our mouths. The partially erupted or unerupted wisdom teeth are called impacted wisdom teeth.
Complications Associated with Wisdom Teeth
- Pain while erupting
- swelling of gums adjacent to the tooth
- tooth decay due to difficulty in cleaning
- Damage to the adjacent permanent molar due to malpositioning of the wisdom tooth
- chronic cheek biting
- severe infections spreading to cheek and neck in case of long-term negligence
- Difficulty in opening the mouth due to pain and swelling
- Development of dentigerous cyst (a fluid-filled cavity in the bone) that grows at the expense of the bone.
Diagnosis (Radiology)
An x-ray reveals the position of the impacted wisdom tooth helping us understand the difficulty of removing it and looking for any associated cyst.

Operculectomy
When the wisdom tooth is straight in position, the gums around the erupting tooth swell and cause pain and food lodgement. In such cases, the adjacent gum tissue can be removed under local anesthesia. This will enable pain-free eruption of the tooth.
Surgical removal of impacted wisdom tooth
Mispositioned, troublesome impacted wisdom teeth should be removed surgically by removing a small amount of bone around the tooth. It is a complication-free procedure.
Dentigerous Cyst Removal
A dentigerous cyst is a fluid-filled sac that develops around the impacted (unerupted tooth) damaging the bone. These cysts usually do not show any symptoms until they get infected or become very large. A dentigerous cyst should be treated surgically (the lining and contents of the cyst are removed). The entire procedure is done from within a mouth – so that there will be no chance of scar formation on the face.
Myths and Facts
Myth: Wisdom Tooth Removal is Harmful to the Brain.
This tooth is called a wisdom tooth only because it erupts after the growth has been completed. It doesn’t have any connection to the brain or eye.
Myth: Unerupted Wisdom Tooth sleeps inside the jaw forever.
Fact: No, even if the tooth is lying submerged completely in the bone, it still carries a significant risk of cyst formation and infection. So unerupted wisdom teeth should never be left untreated.
Myth: Jaw becomes week after surgical removal of wisdom tooth
Fact: Only 1mm to 2 mm bone around the tooth is removed during the procedure. So, there are no chances of jaw bone weakening. However, in the case of deeply submerged large teeth or in case of the large dentigerous cysts, adequate precautions will be taken during and after surgery to avoid weakening of the jaw.
